How do I dematerialize shares I bought with different name and signature on PAN Card?

I had a PAN Card having English signature. But later I changed it to Hindi because of regular complaints of signature mismatch. I also made small verification in my pan name. Now new PAN Card has Hindi sign and changed name issued under same pan number. But I had bought shares of xyz under my old pan details. Now I am not able to dematerialize my shares. What should I do now?
My mom has physical share certificates which were purchased by my grandfather before her marriage. Now I need to convert this shares into demat account but the physical shares are before marriage name and also marriage certificate is not there so how can I convert them into demat form?
Answer:
If you hold shares in physical certificates and the name is different than your current name, you should contact the company or the registrar to change it as soon as possible.
Steps to change the name in the share certificate
Contact the company or send a message to the compliance officer for the exact process.
Contact the registrar of the company.
Fill the application form (Sample) for recording change of name with new signature in the changed name duly attested by Notary / Magistrate under their official seal.
Send share certificates covering the entire holding.
Send one of the following attested copy of the documents:
Marriage Certificate
Gazette Notification
Court order
